新型冠状病毒肺炎胸部CT时间周期与肺部影像变化关联分析

摘要:目的通过观察新型冠状病毒肺炎(Corona Virus Disease 2019, COVID-19)患者各时间周期胸部CT肺部影像变化,研究患者肺部影像变化与胸部CT时间周期的关系。方法回顾性分析94例COVID-19患者在一周内(1~7 d)、两周内(8~14 d)、两周后(>14d)三个周期胸部CT影像变化,并以COVID-19的分型将研究对象分为普通型和重型组进行对比研究。以患者入院前胸部CT肺部病灶情况作为参照,比较入院治疗后各时间周期胸部影像病灶变化及各时间周期胸部CT肺部病灶变化情况进行。结果 94例COVID-19患者入院治疗后各时间周期胸部CT与入院前胸部影像比较,一周内和两周内普通型组与重型组胸部CT变化的差异有统计学意义(P<0.05),其中一周内普通型组好转率27.6%(16/58),重型组好转率16.6%(6/36),OR=1.90。两周内普通型组好转率93.1%(54/58),重型组好转率50.0%(18/36),OR=13.50。入院前后各时间周期胸部CT影像前后相互对比情况提示一周内和两周后普通型与重型胸部CT变化P<0.05,差异有统计学意义,其中一周内普通型组好转率27.6%(16/58),重型组好转率16.6%(6/36),OR=1.90;两周后普通型组好转率10.3%(6/58),重型组好转率88.9%(32/36),OR=0.01。结论普通型新型冠状病毒肺炎患者一周内肺部病灶吸收时间快,重型新型冠状病毒肺炎患者治疗两周后肺部病灶吸收明显,在治疗新型冠状病毒肺炎过程中动态观察影像胸部CT观察肺部病灶变化,能够早期评估治疗疗效,提早干预,降低死亡风险。

Relational analysisof the COVID-19chest CT betweentime period and lung image changes
Abstract:\Objective The COVID-19(Corona Virus Disease 2019) patients were observed to observe the changes of chest CT lung images in different time periods, so as to research on the relationship between lung imaging changes and chest CT time period in patients.. Methods The COVID-19 patients 94 cases were retrospectively analyzed in one week (1~7 days), two weeks (8~14 days), two weeks later (>14 days) three time periods. The chest CT image changes of the three time periods were compared. The COVID-19 patients were divided into general and severe groups. Firstly, using the chest CT lung lesions of the patients pre-hospital as a reference, The changes of chest imaging lesions in each time period after admission and the changes of chest CT lung lesions in each time period were compared. Results Comparing 94 cases of COVID-19 patients with chest CT in each time period after admission and chest imaging before admission, there were significant differences in chest CT changes between the normal group and the severe group within one week and two weeks (P<0.05). In one week, the improvement rate was 27.6% (16/58) in the general group and 16.6% (6/36) in the severe group , OR = 1.90.In two weeks, the improvement rate was 93.1% (54/58) in general group and 50.0% (18/36) in severe group, OR = 13.50. The comparison of chest CT images in each time period before and after admission showed that the CT changes of normal and severe chest within one week and two weeks later were P<0.05, and the difference was statistically significant. During one week, the improvement rate was 27.6% (16/58) in the general group and 16.6% (6/36) in the severe group ,OR = 1.90. Two weeks later, the improvement rate of the general group was 10.3% (6/58), the improvement rate of the severe group was 88.9% (32/36), OR = 0.01. Conclusion The COVID-19 Patients in a week, the absorption time of lung lesions was fast for general type , after 2 weeks treatment the lung lesions absorption obviously for severe type, during the treatment of COVID-19, dynamic images of chest CT were used to observe the changes of lung lesions. So early assessment of therapeutic effect, early intervention, and then reduce the risk of death.
